Caroline serves as the Director of Nursing here at Sun Valley Home Healthcare, where she oversees all aspects of patient care, staff development, and quality improvement. With more than 10 years of experience in the nursing field, Caroline is dedicated to ensuring that every patient receives excellent, compassionate, and individualized care.
Before joining Sun Valley Home Healthcare, Caroline held leadership and clinical roles in home health and acute care hospitals, where she developed a strong background in patient safety, quality improvement, and team leadership.
A passionate advocate for both patients and staff, Caroline believes that exceptional nursing care begins with a culture of kindness, teamwork, and lifelong learning.

Brenna Eppley began her nursing career in 2013 at an inpatient rehabilitation facility, where she provided specialized care to patients recovering from complex injuries, illnesses, and surgical procedures. Her focus on restoring physical function, promoting cognitive and emotional well-being, and fostering patient independence laid the foundation for her ongoing dedication to holistic, patient-centered care.
Throughout her career, Brenna has served in a variety of clinical and leadership roles, including staff nurse, Wound Care Coordinator, RN Manager, and Chief Nursing Officer. In each position, she has championed clinical excellence, quality outcomes, and a culture of compassion, ensuring that patients receive the highest standard of care across all settings.
Brenna holds a Bachelor of Science in Nursing and has been a Certified Rehabilitation Registered Nurse (CRRN) since 2015. Drawing on her extensive background in rehabilitation and her personal experience with home health, Brenna is deeply committed to helping patients recover safely and confidently in their own homes. Her leadership reflects a steadfast belief that exceptional care begins with dignity, empathy, and respect for each individual’s journey to wellness.

Dr. Vicente Figueroa-Diaz is a respected physician and healthcare leader with over 20 years of experience in Internal Medicine, with a focus on Hospital Medicine, Post-Acute Care Medicine, as well as hospice and palliative care. A graduate of Windsor University and former resident at St. Joseph’s Hospital and Medical Center in Phoenix, Dr. Figueroa-Diaz has built his career around compassionate, patient-centered medicine.
As Chief Medical Officer at Desert Valley Hospice & Palliative Care and CEO of Ternion Physician Group, Dr. Figueroa-Diaz leads with a philosophy grounded in kindness, respect, empathy, and integrity. His guiding principles—to listen with compassion, serve with humility, and treat every patient as family—define his approach to care and leadership.
Recognized as the United States Doctor of the Year by Encompass Health in 2015, Dr. Figueroa-Diaz’s commitment to excellence is matched by his unwavering belief that kindness is the foundation of healing.
Beyond his professional life, Dr. Figueroa-Diaz is a devoted husband, father of two, and proud dog owner who lives his values of kindness and care both inside and outside the clinical setting.

Dr. Patrick SciaraAsst. Medical Director
Dr. Patrick Sciara is a board-certified internal medicine physician who has proudly served the Phoenix community for more than 20 years. He completed medical school at the University of Arizona College of Medicine followed by residency at St. Joseph’s Hospital and Medical Center in Phoenix. Dr. Sciara spent many years with Alliance Medical Clinic, where he helped build and strengthen comprehensive primary care services while focusing on accessible, patient-centered care for families across the region.
